A randomized, open label, multicenter Phase 2/3 study to evaluate the efficacy and safety of rogaratinib (BAY 1163877) compared to chemotherapy in patients with FGFR-positiev locally advanced or metastatic urothelial carcinoma who have received prior platinum-containing chemotherapy
Primary Objective:
To demonstrate the superiority of rogaratinib over chemotherapy in terms of prolonging overall survival of urothelial carcinoma patients with FGFR positive tumors
Secondary Objectives:
To evaluate additional efficacy including the following variables:
- progression-free survival (PFS)
- objective response rate (ORR)
- disease control rate (DCR)
- duration of response (DOR)

Part A: Phase 1b
Primary objectives of Part A:
  •  To determine the safety and tolerability of rogaratinib in combination with atezolizumab in patients with FGFR-positive locally advanced or metastatic urothelial carcinoma
  • To determine the recommended Phase 2 dose (RP2D) of rogaratinib in combination with atezolizumab in this patient population.
Secondary objective of Part A:
  • To assess the efficacy of the combination of rogaratinib and atezolizumab in this patient population.
  • To characterize the pharmacokinetics (PK) of rogaratinib in combination with atezolizumab in this patient population
Exploratory objectives of Part A:
  • To assess new potential predictive or prognostic biomarkers and their association with tumor-related biomarkers, disease response, drug-resistance and patient outcome.
  • To evaluate further biomarkers to investigate the drug (i.e. mode of-action-related effect and / or safety) and / or the pathomechanism of the disease.
  • To evaluate the combination of rogaratinib and atezolizumab with regard to patient-reported outcomes (PRO)

Influence of the plant Aristolochia clematitis on the incidence of urothelial carcinoma of upper urinary tract in Germany

Untersuchung von Risiko Faktoren bei urothelkarzinomen des oberen harntrakts. Aristolochia Säure ist ein bekanntes kanzerogen, das urothelkarzinome des oberen harntrakts induziert. Wir wollen untersuchen ob es einen Zusammenhang zur Inzidenz von urothelkarzinomen des oberen harntrakts und auftreten von aristolochia Säure gibt.

Artesunates impact on the progressive growth behavior of therapy-sensitive and cisplatin-resistant bladder cancer cells

Artesunate, eine semisynthetischen Substanz, abgeleitet von dem Naturstoff Artemisinin aus der Traditionellen Chinesischen Medizin (TCM) zeigt auch Potential als anti-tumorale Agens. Da es auch hemmend auf Reparaturmechanismen der DNA einwirkt und damit die Effektivität von Chemotherapeutika wie Cisplatin steigern könnte, soll in der vorliegenden Studie der Einfluss von Artesunate auf ein Panel von Therapie-sensitiven und Cisplatin-resistente Harnblasenkarzinomzellen untersucht werden. Im Rahmen des Vorhabens wird der Einfluss von Artesunate auf das progressive Wachstum, die metabolische Aktivität und die Induktion verschiedener Zelltodereignisse evaluiert. Dazu sind funktionelle Untersuchungen sowie molekularbiologische Analysen der zugrundeliegenden Mechanismen vorgesehen.

Comparison of the genomic nanostructure of therapy-sensitive versus cisplatin-resistant bladder cancer cells

Cisplatin-based chemotherapy is the most common standard treatment for advanced bladder cancer. However, due to resistance development, prognosis remains poor and innovative therapeutic strategies are required. Thus, it is our objective to evaluate malignancy associated properties in a panel of “sensitive” and cisplatin-resistant bladder cancer cells. The genomic nanostructure of the cells and localization of “lead genes” will also be determined.

Preclinical analyses of advanced renal cell carcinoma – Overcoming therapy resistance through innovative treatment approaches – Project 1

Der Behandlungserfolg beim fortgeschrittenen Nierenzellkarzinom (NZK) ist durch die Entstehung von Resistenzen zeitlich limitiert. Zudem wünschen sich insbesondere Patienten im fortgeschrittenen Stadium häufig eine Einbindung natürlicher Therapeutika in Ihre Therapie. Aufgrund mangelnder wissenschaftlicher Daten kann diesem Wunsch selten entsprochen werden. In der anvisierten Studie soll daher der Einfluss von Substanzen aus der traditionellen chinesischen Medizin (TCM) auf therapieresistente NZKs untersucht werden. Im Rahmen des ersten Teilprojektes soll dabei insbesondere die Wirkung auf die progressive Ausbreitung des NZKs in vitro evaluiert werden. Dazu sind funktionelle Untersuchungen des Wachstumsverhaltens sowie molekularbiologische Analysen der zugrundeliegenden Mechanismen vorgesehen. Mittels der Studie soll eruiert werden, inwiefern die Substanzen als Additivum innerhalb der Therapie des NZK sinnvoll sein könnten. Längerfristiges Ziel ist es, eine neue, wirkungsvolle Behandlungsoption für Patienten mit fortgeschrittenen NZK zu etablieren.

Artesunate ist eine semisynthetischen Substanz, abgeleitet von dem Naturstoff Artemisinin aus dem einjährigen Beifuss, der seit ~2.000 Jahren in der traditionellen chinesischen Medizin (TCM) zur Anwendung kommt. Dort kommt er in erster Linie bei Malariainfektionen zum Einsatz, zeigte aber auch anti-tumorale Eigenschaften. In der Studie soll der Einfluss von Artesunate auf therapieresistente Prostatakarzinomzellen untersucht werden. Im Rahmen des Vorhabens wird entsprechend an einem Panel von unbehandelten (=parentalen) und Docetaxel-resistenten Prostatakarzinomzellen der Einfluss von Artesunate auf die progressive Ausbreitung evaluiert. Dazu sind funktionelle Untersuchungen des Wachstums und des invasiven Verhaltens sowie molekularbiologische Analysen der zugrundeliegenden Mechanismen vorgesehen.

The significance of the natural substance shikonin in the therapy of taxane-resistant prostate carcinoma

In der Studie wird evaluiert, ob im Prostatakarzinom durch die Gabe von Shikonin, aus der traditionellen chinesischen Medizin (TCM), eine Therapieresistenz gegen Docetaxel revertiert werden kann. Dabei soll der Einfluss von Shikonin auf das progressive Wachstumsverhalten unbehandelter und Docetaxel-resistenter Prostatakarzinomzellen analysiert werden. In funktionellen Assays wird dazu der Einfluss von Shikonin auf die Proliferation und das Wachstum untersucht, die Induktion apoptotischer und nekrotischer Effekte evaluiert und Verschiebungen in den Zellzyklusphasen sowie zellzyklusregulierenden Proteinen eruiert. Die Analysen sollen darüber Auskunft geben, in welchem Maße die physiologische Balance zwischen wachstumshemmenden und wachstumsfördernden Stimuli in der Tumorzelle durch die Behandlung mit Shikonin wiederhergestellt wird und an welchen Schaltstellen die Therapie ansetzt.

NEUROS: Intraoperative Monitoring of the Pelvic Autonomic Nerves

One of the major problems of rectal cancer surgery is pelvic autonomic nerve damage, which is the main cause of urogenital dysfunction influencing postoperative quality of life. Costs for diagnostics and treatment of short and long-term urogenital dysfunction are immense. Varying degrees of urogenital dysfunction are found in up to 32% and 55% of patients with rectal cancer despite potentially nerve-sparing total mesorectal excision (TME).
The study will examine the impact of a newly developed continuous monitoring device for preservation of urogenital function in patients with TME for rectal cancer. 188 patients will be included in the prospective, randomized, single-blind, parallel group multi-centre trial including two arms (TME with and without intraoperative continuous monitoring of pelvic autonomic nerves). The primary efficacy endpoint is the change in urinary function measured by International Prostate Function Score (IPSS) 12 months after surgery. Genital functions measured as secondary endpoints. The application of the continuous intraoperative neuromonitoring device could enhance the objective intraoperative confirmation of pelvic nerve sparing surgery. The investigators hypothesis is that the use of his device minimizes the risk of postoperative urogenital dysfunction in patients with TME for rectal cancer. An enormous reduction of treatment costs is to be expected.

The relevance of the Akt/mTOR pathway in the pathogenesis of penile cancer

Penile carcinoma is a rare but aggressive cancer with a high incidence in developing countries. Advanced penile cancer is most commonly treated with a combination therapy including cisplatin (CDDP) and 5-fluorouracil (5-FU). However, therapy resistance limits the anti-tumoral effect. Alterations in the PI3K/Akt/mTOR pathway might be involved. Thus, it is our objective to evaluate the relevance of this pathway in penile cancer in vitro and in vivo.
