Primary objectives of Part A:
  •  To determine the safety and tolerability of rogaratinib in combination with atezolizumab in patients with FGFR-positive locally advanced or metastatic urothelial carcinoma
  • To determine the recommended Phase 2 dose (RP2D) of rogaratinib in combination with atezolizumab in this patient population.
Secondary objective of Part A:
  • To assess the efficacy of the combination of rogaratinib and atezolizumab in this patient population.
  • To characterize the pharmacokinetics (PK) of rogaratinib in combination with atezolizumab in this patient population
Exploratory objectives of Part A:
  • To assess new potential predictive or prognostic biomarkers and their association with tumor-related biomarkers, disease response, drug-resistance and patient outcome.

Evaluation of CT-graphical changes during and after polycheomotherypa PEB on pulmonary function in patients with testicular cancer (Bleotox)

Metastasierte Hodentumoren werden entsprechend ihrer Risikoklassifikation mit einer unterschiedlichen Anzahl an Therapiezyklen der Polychemotherapie „PEB“ behandelt. Diese beinhaltet Cisplatin (P), Etoposid (E) und Bleomycin (B). Während für Cisplatin und Etoposid Nebenwirkungen wie die Hyperemesis, Nephrotoxitzität oder Ototoxizität im Vordergrund stehen, ist die Behandlung mit Bleomycin typischerweise mit einer pulmonalen Toxizität assoziiert. Bildmorphologische Veränderungen werden unter einer Bleomycin-basierten Therapie in bis zu 10% beschrieben und in ca. 1% der Fälle kann es zur schwerwiegendsten Form, der sogenannten Bleomycin-induzierten Pneumonitis kommen, die mit einer hohen Mortalitätsrate einhergeht. In retrospektiven Arbeiten konnten Risikofaktoren, die eine Bleomycin-induzierten Pneumonitits begünstigen, identifiziert werden. Hierzu zählen ein erhöhtes Alter (> 40 Jahre) bei Beginn der Chemotherapie, die kumulative Gesamtdosis des Bleomycins und eine eingeschränkte Nierenfunktion. Inwieweit sich die Lungenfunktion unter einer PEB-Chemotherapie mit Bleomycingabe verändert, ist hingegen in der Literatur nur unzureichend beschrieben. Ebenso ist es unklar, inwieweit CT-graphische Veränderungen während und kurz nach der PEB-Chemotherapie mit einer funktionellen Einschränkung der Lungenfunktionsparametern einhergehen.

Expression of novel treatment targets in patients with urothelial cancer and squamous cell differentiation or pure squamous cell carcinoma of the bladder

Plattenepithelkarzinome der Harnblase und Urothelkarzinome der Harnblase mit plattenepithelialer Differenzierung haben verglichen mit einen Urothelkarzinom der Harnblase eine hohe Rezidivrate und eine ungünstigeres Gesamtüberleben. Neuartige Therapien wie zum Beispiel Immuntherapie mit PD-1/ PDL-1 Inhibitoren stellen für dieses Patientenkollektiv weiterhin keinen Standard dar und die Expression dieser Biomarker ist nur unzureichend erforscht.
In diesem Projekt sollen daher neue Biomarker wie der PD-1/ PDL-1 Status untersucht werden und mit den klinischen Daten überprüft werden

Impact of cholesterol- and lipid-homeostasis in patients with advanced and pre-treated prostate cancer (Palltur)

Das fortgeschrittene Prostatakarzinom stellt trotz der Entwicklung neuer Therapieansätze weiterhin eine schwerwiegende onkologische Erkrankung dar und zählt in Deutschland nach dem Bronchialkarzinom zu der zweithäufigsten Krebstodesursache [1]. Die Identifikation neuer Therapie-Targets ist ein wichtiger Schritt, um eine Behandlungsoptimierung zu erzielen und das Überleben von betroffenen Patienten mit fortgeschrittenem Prostatakarzinom zu verlängern.
In diesem Projekt soll in dem ersten Schritt das Gewebe von Patienten mit einem Prostatakarzinom für einen Tissue-Micro-Array aufgearbeitet werden, die gleichzeitig an einer obstruktiven Miktionssymptomatik leiden und hierfür eine subvesikale Deobstruktion mittels transurethraler Resektion der Prostata (TUR-P) erhielten. Dieser Ansatz hat mehrere wichtige Vorteile: Das onkologisch und molekulargenetisch relevante Gewebe wurde im Rahmen einer funktionellen Operation, der TUR-P, gewonnen und der Patient muss sich somit keiner erneuten Biopsie zur Gewinnung von Karzinomgewebe unterziehen. Zum anderen erhielten diese Patienten meist mehrere Therapieregimes zur Behandlung des Prostatakarzinoms, so dass diese klinischen Daten in Hinblick auf Therapieansprechen von großem Interesse sind.
In Zusammenarbeit mit dem Institut für Pathologie soll nun das transurethral resezierte Gewebe von Prostatakarzinom Patienten für einen TMA aufgearbeitet werden. Dieser TMA soll dafür genutzt werden, Expressionsprofile von spezifischen Target-Genen mittels IHC zu entziffern.
Ein Beispiel für ein Target-Gen für das fortgeschrittenen Prostatakarzinom ist unter anderem der Glukokortikoidrezeptor (GR). In den letzten Jahren konnte der GR als vielversprechendes Gen identifiziert werden, das einen Einfluss auf die Entwicklung eines Androgen-unabhängigen Prostatakarzinoms hat [2, 3]. So konnte gezeigt werden, dass der GR im Verlauf der fortgeschrittenen Prostatakarzinomerkrankung und Entwicklung einer Kastrationsresistenz hochreguliert wird.
In Vorarbeiten konnte unsere Arbeitsgruppe zeigen, dass durch die Inhibition des GR mit dem Progesteron- und GR-Inhibitor zu einer Herunterregulation des Cholesterin- und Lipidstoffwechsel kommt und dieser Stoffwechselweg somit einen Einfluss auf die Entwicklung von Resistenzmechanismen einer Docetaxel-basierten Therapie zu haben scheint. Die Ergebnisse decken sich mit früheren Publikationen in denen gezeigt werden konnte, dass Schlüsselenzyme aus dem Cholesterin- und Lipidstoffwechsel, wie den „sterol regulatory elements of binding“, sogenannte SREBs beim fortgeschrittenen Prostatakarzinom eine entscheidende Rolle spielen [4, 5].
Mit Hilfe dieses TMA wäre es möglich, die Aktivität mehrerer in der Literatur beschriebene Target-Gene in einem hochselektierten Patientenkollektiv zu untersuchen. Diese gewonnenen Erkenntnisse sollen ebenfalls mit den klinischen Daten dieser Patienten verglichen werden und z.B. auf Therapieansprechen hin untersucht werden.

KITTU - KI-unterstützte Therapiebegleitung am Beispiel der Urologie“/ AI-supported support in treatment recommendation for cancer patients in urology

In Western countries approximately every fourths new cancer diagnosis belongs to the urinary and genital organs. Recommending the best therapy is complex and has to take into account the individuality of each patients medical history as well as an extensive knowledge that can hardly be surveyed by individual experts. KITTU („KI-unterstützte Therapiebegleitung am Beispiel der Urologie“/ AI-supported support in treatment recommendation for cancer patients in urology) aims to develop an AI- system that supports the treatment of tumor patients in urology by demonstrating physicians and patients each therapeutic option available in their individual situation and supporting them in their decision-making process. KITTU thus contributes to relieving the burden on those involved in the process and to optimizing therapy decisions. With the help of KITTU we aim to increase evidence-based treatment recommendation in oncology in order to improve the quality of treatment and long-term survival of patients. Additionally, preventable side effects should be reduced while and overall quality of life will be improved. Once established in urology, the goal is to extend KITTU to other oncological diseases, thus enabling a significant improvement cancer care.

Growth behavior of bladder cancer after combined application with cisplatin and artesunat

In vorangegangenen Studien konnten wir zeigen, dass Artesunate, eine semisynthetischen Substanz, abgeleitet von dem Naturstoff Artemisinin aus der Traditionellen Chinesischen Medizin (TCM), therapeutische Wirksamkeit sowohl in Therapie-sensitiven als auch Cisplatin-resistenten Harnblasenkarzinomzellen zeigt. In der hier anvisierten Studie wird untersucht, ob Artesunat, zeitgleich mit dem Chemotherapeutikum Cisplatin appliziert, additive oder synergistische anti-tumorale Effekte in den Harnblasenkarzinomzellen zu induzieren vermag. Weiterhin wird in chronischer Behandlung eruiert, ob Artesunat in der Kombinationsbehandlung eine Resistenzentstehung verzögert oder gar vermeidet.

The impact of shikonin combination therapy on the development of therapy resistance in advanced renal cell carcinoma

Shikonin aus der Traditionellen Chinesischen Medizin (TCM) hat anti-tumorale Effekte in parentalen und Therapie-resistenten Nierenzellkarzinomen gezeigt. In der vorliegenden Studie wird untersucht, ob und inwiefern Shikonin, gleichzeitig mit einer etablierten Therapie appliziert, eine Therapieresistenz zu vermeiden oder verzögern vermag. Im Rahmen des Vorhabens wird entsprechend der Einfluss einer kurzzeitigen versus einer chronischen Kombinationstherapie auf das progressive Wachstumsverhalten der Nierenzellkarzinomzellen im Vergleich zu den jeweiligen Einzelbehandlungen evaluiert. Hierzu sind funktionelle Untersuchungen sowie molekularbiologische Analysen der zugrundeliegenden Mechanismen vorgesehen.

50 Patienten mit einer uroonkologischen Diagnose (Prostatakarzinom, Urothelkarzinom, Nierenzellkarzinom) und unter palliativer medikamentöser Tumortherapie (Antihormonelle Therapie, Chemotherapie, Immuntherapie, Target Therapy) wird vor der Boosterimpfung, 4-8 Wochen nach der Booster-Impfung und 6 Monate nach der Boosterimpfung 1 Serumröhrchen (7,5ml) im Rahmen der routinemäßigen Blutentnahme abgenommen für die Anitkörperbestimmung (IgG Anti-Spike-Protein). Auch werden folgende Daten erhoben: Persönliche Daten, Diagnose, Therapie, Datum der abgeschlossenen Erstimpfung, Datum der Boosterimpfung, Impfreaktionen, Nebenwirkungen der Tumortherapie.

The relevance of the Akt/mTOR pathway in the pathogenesis of penile cancer

Penile carcinoma is a rare but aggressive cancer with a high incidence in developing countries. Advanced penile cancer is most commonly treated with a combination therapy including cisplatin (CDDP) and 5-fluorouracil (5-FU). However, therapy resistance limits the anti-tumoral effect. Alterations in the PI3K/Akt/mTOR pathway might be involved. Thus, it is our objective to evaluate the relevance of this pathway in penile cancer in vitro and in vivo.
