Genauigkeit der chirurgischen Übertragung mit 3D-basierten Schablonen

Die dreidimensionale Erfassung des Knochenangebotes für enossale Implantate erlaubt mit Hilfe von Planungssystemen eine virtuelle Implantatpositionierung und über 3D-basierte Schablonen bzw. spezielle Equipements einen Transfer des virtuellen Plans auf den operativen Situs. Vorteile dieses Verfahrens sind u.a. in einer exakten Übertragung der prothetischen Zielsetzung, einer Verringerung des Risikos der Verletzung anatomischer Strukturen und einem vermehrten Einsatz minimal-invasiver Operationstechniken zu sehen. Ziel des Projektes ist die Prüfung der Genauigkeit der chirurgischen Übertragung für die Insertion dentaler Implantate basierend auf 3D-Planung und Umsetzung mit Hilfe differenter statischer Navigationssysteme. Hierbei werden verschiedene Einflussfaktoren geprüft, wie z.B. die laborseitige bzw. nach dem rapid prototyping Verfahren erzeugte Herstellung der Schablonen sowie differente Planungs- und Transfersysteme. Lage- und Winkelabweichungen zwischen geplanter und reeller Implantatposition werden nach Segmentierung und Fusion des prä- und postoperativen DVT-Datensatzes bestimmt.

Fourier-based automated detection of structures in radiographs

Development of a test apparatus for implementation of material testing on endodontic files

The intent of the project is the development of a preferably variable meter prover  to perform different material tests on endodontic files. Therefore a collet chuck adjusting the files is added to a freely programmable stepping motor adjustable in all dimensions through a high-precision variable tabl. On the opposite side, the files are closely fixed by a clamping jaw and destroyed by the rotation of the motor. The file is part of a monitoring current circuit which fails when the file is broken. A special software registers the failing oft he monitoring current circuit via an experimental interface board connected via usb to a PC. The software can also control the setting of for example the motor-speed and traversing angle. The complete apparatus is constructed sufficiently variable to allow potential upgrades in any part of hardware and software easily.

Entwicklung einer Testapparatur zur Durchführung von Materialtests an endotonischen Feilen

Das Vorhaben hat als Ziel eine möglichst variable Testapparatur zu bauen, die verschiedene unterschiedliche Tests an endodontischen Feilen durchführen kann. Hierzu werden ein frei programmierbarer Steppermotor, welcher durch Präzisionsverstelltische in alle Raumebenen verstellt werden kann, mit einer Spannzange versehen, um die Feilen einzuspannen. Auf der gegenüberliegenden Seite werden die Feilen durch eine Spannbacke fest eingespannt und durch Rotation des Motors zerstört. Die Feststellung des Defektes erfolgt über einen Überwachungs-Stromkreislauf, welcher über die Feile geführt wird und der durch Bruch ausfällt. Dies wird über eine Testplatine erfasst und an den PC weitergeleitet. Über eine Software kann sowohl der Feilenbruch festgestellt, als auch der Motor selber in vielen Variablen wie Geschwindigkeit und Verfahr-Winkel programmiert werden. Die gesamte Apparatur ist ausreichend dimensioniert und variabel, sodass auch eventuelle Upgrades leicht zu realisieren sind.

Implementierung eines longitudinalen Curriculums zur Verbesserung der Kommunikationskompetenz von Zahnmedizinstudierenden

Die Studierenden sollen die herausragende Bedeutung des ärztlichen Gesprächs auch im Rahmen der zahnärztlichen Behandlung erkennen. Dies bezieht sich sowohl auf die anamnestische Befragung aber auch die Erläuterung und das Umsetzen therapeutischer Maßnahmen. Da sich die Zahnmedizin inzwischen als integraler Bestandteil der Medizin versteht, müssen dem heranwachsenden Nachwuchs auch entsprechende ärztliche Fähigkeiten vermittelt werden. Die im Rahmen des ärztlichen Gespräches zu erzielenden Placebo- und vor allem Noceboeffekte müssen den Studierenden theoretisch dargestellt, von diesen praktisch erprobt und ihnen bewusst gemacht werden.
Ein weiteres Lernziel sollte das Erlernen unterschiedlicher Kommunikationsstile sein. Entsprechend der Situation und der geplanten Behandlung bzw. dem vorliegenden Krankheitsbild und der Persönlichkeit des Patienten sind verschiedene Kommunikations¬techniken möglich und nützlich. Dabei müssen selbstverständlich auch die medicolegalen Empfehlungen (z.B. bei der Aufklärung) berücksichtigt werden.

Differentiated comparison of the effect of articaine in pulpal and soft tissue anesthesia depending on the concentration of the vasoconstrictor epinephrine using infraorbital nerve block

A comparison of the efficacy of a local anesthetic used in dentistry, Articaine, depending on the added epinephrine concentration concerning onset, depth and length of dental pulp and soft tissue anesthesia. Dental pulp anesthesia will be objectified by using an electronic pulp tester, soft tissue anesthesia will be rated by the subjects. The infraorbital nerve block being the used injection technique, it will affect the anterior teeth and premolars of the upper jaw as well as the soft tissue of palate, cheek and upper lip of the injection side. As a feasibility study,  we try to evaluate the infraorbital nerve block as a standardized method for clinical trials concerning nerve block anesthesia.

The Placebo effect in the context of topical anaesthesia in dentistry

About 60% of the population are afraid of painful dental treatment or of dental injection. One possibility to make an injection more comfortable is topical anaesthesia. Lidocaine is a well-known and established topical anaesthetic agent in dental treatment. In addition pain can be influenced by placebo mechanisms such as experience, expectation or classical conditioning. It is known that the pharmacological effectiveness of pharmaceuticals can be boost up to 50% by using placebo mechanisms. The aim of the study is to constitute the consequence of information (positive/neutral) to the pharmacological effectiveness of lidocaine/placebo on the oral mucosa in the context to open-hidden-paradigm in dental anesthesia under the influence of dentist-patient-relationship.
In an experimental conditioning design (8 groups) we are testing 88 healthy volunteers (41 male, 47 female) in a double blind, randomised trial. Participants´ anxiety and behaviour to pain perception as well as probands’ anxiety and pain behaviour are checked with special questionnaires (MDAS, STAI), for self-estimation we use visual analogue scales (VAS). We investigate the change of pain sensitivity provoked by PinPrick instruments on the oral mucosa (frontal vestibule of the mandible) according to the factors substance (placebo/lidocaine), verbal instruction (positive/neutral) and open/hidden application (eye mask) among base- and testline. The verbal instruction is reinforced by the conditioning stage. Participants who are instructed positive, experience a reduction of pain in PinPrick-testing, unbeknown that the intensity of pain stimulus is reduced. PinPrick-rating is documented on a visual analogue scale. We use ANCOVA for statistic analysis and investigate the data of the collective as well as split up under the focus of dentist-patient-relationship.

Automated detection of (unintended) patient motion in dental Cone Beam CT (CBCT)

Similar to conventional Computed Tomography (CT) dental Cone Beam Computed Tomography (CBCT) generates 3D-data by means of (filtered) backprojection from a multitude of two-dimensional (2D) projection radiographs acquired in a circular arrangement around the patient’s head. Due to the construction of the devices, this image acquisition takes several (commonly > 10s) seconds. As a consequence, pa-tients frequently move slightly during this period resulting in a decreased image quality. One typical effect is motion blur that reduces the effective spatial resolu-tion of the images. This project aims to automatically detect such motion in the 2D-projection images. CBCT-data produced for pre-operative planning of guided implant insertion require extra high spatial resolution. Commonly the radiographic splints applied for such planning integrate (spherical) radio-opaque markers for re-gistration of the 3D-data to the patient coordinates. Our project starts using the spherical marker projections on the 2D projection radiographs. In addition, general marker-free approaches for motion detection and quantification will be developed.
